Do you have standard Paramount insurance?


Exam

Most patients who have Paramount have an eye examination benefit included in their plan every 366 days. This covers routine care. Paramount as a medical plan covers infections, ulcers, foreign body removal, red eyes, dry eyes, and other medical problems whenever they occur (not subject to the366 day rule). We will call to verify that you are eligible, and, if so, your will only be responsible for your specialist copay for the exam.


Glasses

Many patients who have Paramount have coverage for glasses either via a rebate from Paramount, or a subcontract through Superior Vision Plan. We are also a direct provider for Superior Vision! So your glasses benefits could be used here as well!

 

Do you have Paramount Advantage Insurance?


Exam

Paramount Advantage will cover your exam in full every 366 days.

Glasses

Paramount Advantage will cover one pair of glasses every year for children 18 and younger, and one pair of glasses every two years for those patients over 18. At Northwest Ohio Eyecare, we allow our patients with Paramount Advantage to pick from ANY frame priced $74 or less, or apply the $74 towards the purchase of a different frame. Basic plastic or polycarbonate lenses are covered in single vision or standard lined bifocals. Upgrades can be made for additional costs.

 

Do you have Paramount Elite Insurance?


Exam

Paramount Elite allows for one exam every 366 days. This would cover a routine vision check-up. In addition, your insurance will cover medical based visits here as needed. So, if you are experiencing any eye discomfort, redness, dryness, flashes or floaters, your insurance will cover your visit. You will only be responsible for your specialist copay.


Glasses

Most patients who have Paramount Elite now have glasses coverage! This is a new service subcontracted through Superior Vision Plan. It covers $75 toward a frame, and covers basic plastic bifocals. Upgrades are available at 20% off retail prices.
